The Proposal Form
Students must submit a Community Service Proposal Form annually, or whenever they begin a new service project. Be sure to fill in the form completely using your full name (no nicknames, please!) and have it signed by your parents after they have reviewed your project and service site
Note: The only events or projects that do not require Proposal Forms are those that are sponsored by and IHHS faculty member or club such as Heal the Bay, or the AIDS walk.

The Evaluation Form
Evaluation forms must be submitted for each event or project that you complete. There are three due dates during the school year that will be announced. Service completed within a given time period must be documentd by a completed service evaluation form. Be sure to print the form and take it with you to the event or to your last service session so that your site supervisor can sign it right away.

Due Dates 2012-13
The following are upcoming due dates for Community Service Project Proposals and Evaluations. Please note that this year, there are cut-off dates for submitting paperwork for definite, designated time periods.
Forms may be submitted at any time up to the final due date. Don't wait until the due date to complete your paperwork! All forms should be turned in to your theology teacher.
Students who are not in a theology class this semester will by contacted by Ms. Pollia through school email.
Monday, March 25, 2013- This is the last day to turn in Community Service Evaluation forms for service projects completed between
January 7th and March 26, 2013 This is the Final Deadline for the 2012-13 school year!
You must turn this evaluation in even if you will continuing your service at the location after March 30th.
Any service hours earned after March 30th will be credited toward the 2012-2013 school year.
